Registration

Below you will find the forms needed to enroll in the Portland Partnership for the 2019-2020 school year.

• An enrollment form needs to be filled out for EACH student. This includes both new and existing partnership students. A signed enrollment document will be mailed out and will need to be returned before school begins.
• A class registration form will need to be filled out as soon as possible. As soon as you know you classes for the fall
• New partnership students only: A copy of immunization records or a waiver for must be faxed or emailed (fax number and email are listed on the home page) as soon as possible.
• New partnership students only: A copy of birth certificate must be faxed or emailed (fax number and email are listed on the home page) as soon as possible. We will be hosting a mandatory partnership meeting in August in which we will need to see an original birth certificate.
• New and returning students: A proof of residency must be emailed or faxed (fax number and email are listed on the home page) as soon as possible. This can be anything that shows your current address; a piece of mail, a bill, etc.

Portland Partnership Class Registration Form - 2019-2020

• Elementary students (K-5) can take 1 class; Middle School students (6-8) can take 3 classes; High School students (9-12) can take 5 classes. No more than two of the same type of class; music, PE, art, etc.

Birth Certificates

You may email a copy of student birth certificates to portlandpartnership@portlandk12.org. Please include parents full name and students full name in the email. I will need to see the actual birth certificate with a state seal before classes begin.
