leave requests

Please complete the following forms to notify of any absence/s for your daughter due to illness, departure during the day for medical appointments, late arrival at school, extended absences for travel or participation in external events.

• For planned absences of LESS THAN 5 days, please complete the ‘Request for Leave’ form. 

• For extended planned absences, 5 DAYS OR MORE please complete the ‘Application for Extended Leave – Vacation & Travel’  form and forward to principal@plcarmidale.nsw.edu.au

• For extended absences to attend a sporting or arts event, 5 DAYS OR MORE, please complete the 'Application for Exemption from Attendance at School for Elite Sport/Arts Program Participation' and forward to principal@plcarmidale.nsw.edu.au
