A few forms are required of all summer program students (YSI, ISP, or CE).
Please fill out the below forms by their due dates.
Due: Friday, May 15
Form 1: Summer Info Form (digital)
This form includes information on your name, T-shirt size, and travel information if available. This form takes one minute to complete.
Due: Sunday, May 31
Form 2: 2026 Summer Required Forms (digital)
This form includes the Handbook Acknowledgement & Release Form, Vehicle Release Form, Medical Release Form, Insurance Information, and Emergency Contact Information. This form will take approximately 15 minutes to complete. Please have your medical insurance card accessible when completing this form.
Due: Sunday, May 31
Form 3: Physical Exam Form (physical form sent via email)
This form must be printed and signed by a physician (PCP, MedExpress, etc.). Once completed, please scan and email the completed form to the School Office (pbtschool@pittsburghballet.org)
For those staying in the dorms at Chatham University, if you are interested in having an air conditioning unit being installed, you must provide a doctor's note (separate from their physical exam form) explaining their medical need for the AC unit. An AC unit installation surcharge is given by the dormitory, and this fee is then passed on to the families through their portal account. Medical notes should be emailed to the School Office and noted in the email. Following receipt of the medical note, the $125 surcharge will be applied to the account.
Q: Does everyone need a physical? Is the medical form just for the AC unit?
A: Every student needs to submit the physical form and the medical form. If students are interested in acquiring an AC unit, they must send in an additional note from their doctor to pbtschool@pittsburghballet.org.
Q: Can my physical be enough to get the AC unit?
A: No. Students are required by the university to submit a separate doctor's note to acquire the AC unit.
Q: Why do I need to have a separate note for the AC unit? Why does it cost money?
A: These instructions, and the cost, come directly from the university. Without these steps being taken, an AC unit will not be installed.
Q: Do I have to be seen by my PCP? Can I go to any doctor to get a physical?
A: Students may be seen by any medical professional who can give a 'sports' physical. This can be their primary care physician, a doctor from a walk-in clinic, or another doctor.
Q: If both my roommate and I want/need an AC unit, do we both need a doctor's note?
A: As long as you have received confirmation that you are going to be roommates, only one student will need to submit an AC note/cost. Please note that we try to honor as many roommate requests as possible, but this may not be guaranteed.
