Need to make a referral to Phoenix Domus Men's Halfway House? To begin the process, email: referrals@phoenixdomus.com. Once you email, our dedicated and friendly referral/intake staff will work with you through the entire process of making the referral to and through admissions to our facility.
Initial Referral Process:
Initial inquiry (email to our facility)
Interview with the Person in Recovery (Case Managers/Counselors/other Discharge Staff may participate)
Items needed for admissions:
Physical from facility Physician/CRNP/PA (within the last 6 months)
Letter physically clearing PIR for transfer to 3.1 Level of Care (Signed by Physician/CRNP/PA) - (Embedded below)
Biopsychosocial completed at sending facility (within the past 6 months)
Discharge/Transfer ASAM (completed at/near time of discharge from your facility)
MARS (with complete/current medication list at time of discharge)
Getting the above information to us in a timely manner is critical to us being able to provide seamless and individualized care to our shared consumers. As well, it will ensure we are in compliance with various regulatory oversight agencies.Â
