Initial Referral Process:

1. Initial inquiry (email to our facility)

2. Interview with the Person in Recovery (Case Managers/Counselors/other Discharge Staff may participate)

Items Mandatory for Admissions:

1. Physical from facility Physician/CRNP/PA (within the last 6 months)

2. Letter physically clearing PIR for transfer to 3.1 Level of Care (Signed by Physician/CRNP/PA) - (Embedded below)

3. Biopsychosocial completed at sending facility (within the past 6 months)

4. Discharge/Transfer ASAM (completed at/near time of discharge from your facility)

5. MARS (with complete/current medication list at time of discharge)

6. A 30-day supply of all client current prescribed medications OR refills for all currently prescribed medications, signed by Physician, PA, or CRNP.

Getting the above information to us in a timely manner is critical to us being able to provide seamless and individualized care to our shared consumers. As well, it will ensure we are in compliance with various regulatory oversight agencies. We will not be able to admit someone without all of the items listed in 1-6 above.
