A valid learner’s permit is required to be added to the email list. DO NOT COMPLETE UNTIL STUDENT HAS PERMIT!
Completing this form does not guarantee a spot in the class.
Being on the information list is not class registration — it simply ensures you’ll receive registration details when enrollment opens.
You must be a student in the Philomath School District in order to register for this course.

Limited Spots Available: There are always more students on the email list than available spots in the class. Because of this, spots cannot be held or reserved for any reason.
First-Come, First-Served: Once registration opens, those on the information list will receive an email with registration instructions and the required forms. Enrollment is only finalized when payment is received — submitting a completed form and permit alone does not secure a spot.
⚠️ Please do not share registration forms — only students on the information list are eligible to register.
