For me as the nurse to administer medication (including epipen and asthma inhaler), an MED-1 form needs to be completed by a physician, nurse practitioner, or physician's assistant, then returned to the nurse with the prescribed medication in the original pharmacy box. I cannot accept medication that is unlabeled, out of packaging, and without MED-1 form.
