HEALTH FORMS
Please see Requirements by Grade to find out what your child needs for the school year based on their grade.
Must be completed at the start of every school year.
This form provides emergency contact information to the nurse, brief medical history of student, and
consent to administer tylenol and ibuprofen.
Without consent, the nurse can not administer medication to a student.
The nurse is no longer administering covid tests for students with symptoms, at-home tests are available and will be sent home with students as needed
HEALTH SCREENINGS
Health screenings will begin in October of each school year.
Every student's height, weight, and vision will be screened.
All 6th and 7th grade students will be screened for scoliosis.
All 7th and 11th grade students will be screened for hearing.
Additional screenings may be completed upon request by parent or teacher at any time during the year.
If you would like to opt your child out of screenings, please notify me.
