Recommendation Letters

Complete this during the spring of your Junior year.  The form below must be completed and will only go to your school counselor. Be sure to make a COPY of the form to submit.  

• If your counselor is Mr. Santangelo, please fill out this form.             

• If your counselor is Ms. Blau, please fill out this form. 

• If your counselor is Mrs. Fitzpatrick, please fill out this form.

• If your counselor is Ms. O'Connor, please fill out this form. 

You will also need to ask two teachers (preferably core class teachers) to write a letter of recommendation for you. You can ask teachers you feel you have a good relationship with or ones that you may not have done so well at the beginning of the class and worked really hard to bring your grade up. 

Please fill out the form below for your teachers to write your letters of recommendation. You should receive your responses in your school email, in which you will need to forward to your two teachers that you have asked to write a letter for you. Please ask your teachers to write a letter for you before sending them this information.
