COMMITTEE FORMS - Electronic version of these forms should be emailed to the treasurer and president.
Booster Deposit Form - Turn in this form at the end of your event/fundraiser along with any checks or cash collected. Electronic version of the form should be sent to the treasurer and president.
Cash Box Request - Use this form to request start up Cash for your event or fundraiser. Send a minimum of one week before your event
Invoice/Reimbursement Form - fill this out to request payment to a vendor or to request reimbursement. Receipts or an invoice are required.
MARCHING BAND SIGN UP FORMS for 2024-2025 (complete each year)
MB Member Data Form and Agreements 2024-25 (1 for each student)- used only by the Boosters for communication purposes, sending student account statements, determining end of year awards, uniform records, and emergency contact during Booster events.
Medical Form 2024-25 - email to fhebenstreit@pennsburysd.org or print and place in Mrs. Hebenstreit’s mailbox in the music suite at PHS East
Booster Volunteer Form 2024-25 (1 per family) - volunteering is the best way to get involved and feel like you are a part of this great community! We need your help!
