To complete the enrollment process at Pender High School, the following items must be obtained and uploaded before your student can be enrolled:
Withdrawal form from previous school (for in-county transfers)
Birth Certificate or Mother's Copy
Proof of parental rights or custody (parent's name listed on birth certificate or a court order granting custody). A custodial guardian must be present to enroll the student.
Proof of your residence: a lease, contract, or deed. We also accept utility bills should the other forms of proof of residence not be available. If you are living in a home where you are not the owner or renter, you must complete the Residential Affidavit (this form must be signed in front of a Notary Public by the owner of the residence).

Immunization Record
Current photo ID of parent/guardian

Pender County Student Information Form
Medical Alert Form
McKinney-Vento Form
Home Language Survey
National School Lunch Program Form (even if you don't think you qualify)
Bus Transportation Form
