There shall be a complete physical examination by a licensed physician, physician assistant, or advanced practice registered nurse of each child prior to or upon first entry into the public school system and thereafter as often as deemed necessary by the local school authority.

Sports physicals will be provided at school on dates TBD with the assistance of local health providers.
Children must have proof of all required immunizations, documentation of immunity, or valid exemptions, in order to be admitted or enrolled in any school in New Hampshire. Please provide documentation to the Health Office for any new immunizations that your student may receive.

The state of New Hampshire does not mandate health screenings by school nurses at this time. However, many nurses choose to perform screenings in an effort to improve the health and well-being of their students
We have the ability to conduct vision screenings in our office and will make necessary referral recommendations as needed.
We also utilize the local Lions Club once a year to conduct vision screenings with the use of a Welch Allyn Spot Vision Screener [Automatically capture an image from a non-invasive, three-foot distance using the fixation target and sounds that help keep a child’s focus. A bright touchscreen displays results quickly, indicating whether measurements are in range or a complete eye exam is recommended]. This service is free but requires parental consent to be completed (permission slips sent home prior to screening event.)
The Office of School Health recommends a complete vision exam for all students within the first year of entering school or transferring to a new school. Complete vision exam includes near, distance, color and binocular vision. If the physical exam documentation completed by the physician includes vision screening results, the school nurse does not need to repeat the screening. Periodic rescreening for distance vision only is also recommended. Consider vision evaluation for any complaints of headache or visual problems and when referred by school staff.

Our dental hygienist, Ruth, makes biannual visits to the schools in our district. Her services require a parental consent to be completed (permission slips sent home prior to screening event.)

Dental cleanings and sealants on a sliding fee scale
Restorative care for children who are in pain from decay and infection
Case management and financial assistance are available to those in need.
