Hotspot Request Form

Below, you're provided with two options for accessing our hotspot request form: electronic copy and hard copy. Please read the directions before accessing either form:

• Hardcopy Form: Link

Directions for Electronic Copy:

• Click on the link provided

• Download the .pdf

• Fill the .pdf out by clicking in the boxes provided

• Complete the document by providing required information

• Once the .pdf is filled out, attach the document and email to the following address: thomas.foster@pecps.k12.va.us

Directions for Hardcopy:

• The hardcopy can be downloaded by accessing the link above

• Once downloaded, print the copy

• Complete the document by providing required information

• Drop copy off at your child's school

If neither of these options work for you, please visit your child's school and request a hotspot form from the main office. A personnel member will be happy to assist you.
