All SDD collected through this form will be used for monitoring and evaluation (M&E) of all PCAARRD programs, projects, and activities. This data will provide helpful insights to help project designers and implementers develop interventions that are responsive to the needs of target beneficiaries by considering their needs, attitudes, and behaviors.

We aim to fully comply with RA 10173 also known as the Philippine Data Privacy Act of 2012. The responses will only be accessible to the members of GADSPU-SERD and will be used only for enhancing and tailor-fitting DOST-PCAARRD's programs, projects, and activities. All data will be anonymous and no personal data that can be attributed to an individual will be shared.
