In order for any medication to be administered at school by the School Nurse, a written physician order must be completed by the doctor and signed by a parent/guardian.
The medication must be delivered to the School Health office by an adult in the original prescription container with the correct prescribing information on the label.
A new form is needed for each different medication and needs renewed each school year.
If you would like your student to be able to take OTC (over the counter) medications at school, we ask that you complete and return the OTC medications form and return them to your child's school nurse as soon as possible. The parent request form will allow the school nurse to give your child some basic OTC medications as needed at school. It is optional. This consent form will only be good for ONE SCHOOL YEAR.
Request for OTC medications form: Please initial in the right hand column box for each medication that you would be willing to allow the school nurse to administer to your child. Only the OTC medications that you consent to may be administered to your child. Also, please note the following:
