Our role as the School Counselors' at Southmore Intermediate begins and ends with the positive connections made. We strive for the success of all students which encompasses the whole child. Our "Success...whatever it takes" motto recognizes the need to not only focus on the educational world, but the social and emotional world of our students, staff members, and community. Through our systems of great collaboration and connections to resources needed in everyday life, we will create systematic change setting our kids up for success in life.
This form can be used by students, needing to request a visit to a counselor for support. Your counseling team members are available to assist you through phone call, email, or video conference. Please give as much detail as possible so that we can schedule you accordingly. If this is an emergency, please call 911.

This form can be used for PARENTS or TEACHERS to Schedule a Parent/Teacher Conference. Please indicate what teacher(s) you would like to have participate in the conference.
Parent conferences are typically scheduled before school starts, at 7:30AM this way the team of teachers will be available. However, we can schedule individual teacher conferences around teacher conference time as needed.
