• Please be advised this must be accompanied by a Dr's Certificate or other evidence of illness/misadventure (See page 2) and must be submitted on the first day that the student returns to school.

• If a student has Covid or is a close contact - parents and students are able to obtain a Dr's certificate via a telehealth appointment.

• The student must still complete any outstanding assessment tasks on their first day back.
