Classroom Application Process

1. Print, complete (upper and lower sections),  and sign the application form below 

2. Scan and email the document to Kira Brennan. 

3. Cards will be created and send out in 1 to 2 weeks 

*This replaces the practice of having to go to a SLCL branch to complete the application and show proof of employment.

** The card number field is SLCL and can be left blank.

*** SLCL will look the teachers up to see if they already have personal cards. If they don't, they will create personal card as well as a classroom card.
