For Applicants who come from Schools in the Philippines

1. PSA authenticated Birth Certificate

2. Baptismal Certificate

3. Parents' Marriage Contract

4. Report Card of the last school year completed

5. Duly filled-in Parents' Information Sheet

6. 1 pc. 1x1 colored picture of your son

7. Certificate of Good Moral Character (for high school applicants)

For Applicants who come from Schools Abroad

In addition to the requirements stated above, please submit the following documents:

1. Original and photocopy of passport (an original copy is needed for authentication purposes)

2. A ‘Red Ribbon’ certification of the school records secured from the Department of Foreign Affairs

3. Results of any standardized test taken at his former school with test interpretation and the norms used

4. Transcript of Records with explanation of the marking system

5. Two letters of recommendation from the school last attended (i.e. Principal and Homeroom Teacher or Guidance Counselor)

1. Fill out the form with the information required.

2. After submitting the form, wait for the confirmatory email from the Accounting Office for instructions on the mode of payment.

3. Submit a copy of the proof of payment stating the name of student, grade level and reference number to accounting@parefspringdale.edu.ph.

4. Once proof of payment is received the Accounting Office will send a confirmatory reply stating your official enrolment to S.Y. 2022-2023.

Nota Bene:

*Arrangements for book payments and required school materials shall be emailed to the parents soon. Parents shall be informed on the modes of receiving the book requirements for the coming school year
