Intramural Sports & Ski Club Participation Forms

Please Note: All intramural sports and ski club clearance procedures are no longer handled through FamilyID. All registration & clearance procedures are now handled through the Genesis Parent Portal.

You must bring the entire Preparticipation Physical Evaluation Form (see below)  with you to your appointment with your healthcare provider. Only the last page needs to be submitted to the school nurse for clearance.

Ski Club - Please note, registration through Genesis does NOT guarantee a spot in Ski Club. Students will be selected to participate in Ski Club through a lottery process this year, it will not be on a first come first serve basis. 8th graders get priority for entrance into the club, followed by 7th and then 6th graders if spots are still available.
