Absence Excuse Form (2026-27) - Students will use this form to communicate all future absences. It must be turned in at least two weeks prior to any expected absence. Students should also speak with Mr. Moore and send an email too.
Chorus Date Conflicts (2026-27) - After reviewing the calendar, please list all unavoidable conflicts on this form.
Men's Uniform Questionnaire - We need the shoe size and jacket size for all the boys. Shoes are $45; Jackets will be provided by the choir.

Form 2680 - Parent/Guardian Consent and Responsibility Statement for Student Participation in Events/Activities. Required for students to participate in after school rehearsals and performances.
