Please provide the following:

• Please alert us to any medical conditions or allergies your child has. 

• The Tdap vaccine is required for all students who are 11 years or older entering 6th grade. Please be sure your child has this specific vaccine and to schedule an appointment with their provider as needed prior to school starting. 

• An over-the-counter medication form optional but if you would like a student to receive these medications at school it should be signed by both a parent and the child's physician.

• Please make sure all the information on Schooltool is updated with emergency contacts.
