Absence Request

Complete the following form prior to any absence from band rehearsals or performances. All absence requests for performances (concerts, parades, football games, etc.) must be received no fewer than 5 days before the anticipated absence. In the case of an unexpected absence (such as illness), please submit a form within 3 days of the absence.
