Practicum Place of Work Proposal Form

This form is designed for students to propose their place of employment as a practicum field site placement. The approval process will require the proposed field instructor, employment supervisor, workplace administrator, and Director of Field Education to review and approve prior to the student being placed.

*Note: The Administrator with fiscal authority could be, for example, a member of the organization's Human Resources team, the company's Executive Director, or your Field Instructor. We recommend students ask their field instructor who has fiscal authoritative responsibility at the field site prior to starting this form. This ensures the student successfully assigns this form to the right members of their team.
