General Information Forms

Family ID Registration Information - All student-athletes must complete the Family ID Registration to be eligible to compete in a Oswego City School District sport.

Impact Testing Form - Student are required to complete an impact test, which is a tool to indicate if a concussion has occurred.

Skin Infection Form - For students who visit the doctors with that possibility of having a skin condition.

Travel Release Form - For students receiving a ride home from an individual other than parent/guardian. (Must be completed 24 Hours prior to contest.)

Mix Competition Form - For female students, who are interested in competing in a predominately male sport.

Athletic Placement Process - For students at the middle school level, who would like to compete at the Junior Varsity/Varsity level.

Donation Letter - A generic letter to inform businesses, organizations, and corporations of our cause and the purpose of the donations we receive.
