OSU FST has made available the missing instructor documents from the Oklahoma State Department of Health EMS Division Website. 

Below is the login information to gain access to the forms.

The forms and documents will be available while OSDH EMS updates their website.  Please feel free to share this information with others who may need to access these forms.

The documents are housed in IFSTA Resource One through OSU FST.  Please see the access instructions below.

3) At this point, you will be prompted to enter the enrollment key for your course. If you do not see where to enter the enrollment key, click the above link to your course again and ensure you are logged in.
4) You are now enrolled. You can return to this course by using the "My Courses" link at the top right of the page.
