Here are some helpful tips to better prepare for your visit:
Confirm your Appointment.
When your appointment notification is first issued, you *must* confirm your appointment with our Scheduling Coordinator before the specified "Confirm By". Failure to do so, will result in the appointment being cancelled.
Check your email.
Make sure to check your email before your appointment just to make sure your appointment is booked. We will send out a reminder by email 48 hours prior to your appointment. The email will contain important information regarding your appointment. If you do not see an email reminder from us, check your spam/junk folder or contact us.
You may also get a text message reminder if we have your cell phone number on file.
Know where you are going.
Its best to use our provided directions so you won't have any trouble finding our clinic. We will send you a copy of the directions with your confirmation as well with your appointment reminder.
Currently we have two clinic locations. Even though you may have seen Dr. McGaw at one location, does not mean you will see him at the same location. Make sure you check the notice and reminder to know which location your appointment is at.
Give yourself plenty of time to get there.
The Southgate clinic is near the L.R.T. line. During peak periods (like rush hour), you may be held up in traffic because of the continual trains.
Arrive Early.
Dr. McGaw is generally running on-time. It is important that you arrive 10 minutes early (unless otherwise noted).
Come Prepared.
When you receive your Notice of Appointment, it will specify if you are required to bring anything to your appointment (like x-rays or list of prescribed medications). Please don't forget to bring your Health Card.
If this is your first visit, be prepared to complete a medical questionnaire. You can download the medical questionnaire, print and complete the form ahead of time and bring it to your appointment to make your experience quicker.
