When submitting a new Patient Referral, we would ask that you use the latest Referral Request Form from our office.

Please ensure that you are using the latest forms. Dr. McGaw no longer sees patients at the Kaye Edmonton Clinic or University of Alberta Hospital.
When completing the Referral, please remember:
Always use a pen (never pencil). Please print. If you cant read your own writing (we cant either), so its best to use the Enhanced Form.
Your referral must include the Referring Doctors Practitioner ID number (also known as a PracID).
The Alberta Health Services "PracID" number of the referring clinician. If you do not know/have your PracID number, it can be obtained by completing and signing the Practitioner ID request form which is available from the AHS Website: https://formsmgmt.gov.ab.ca/Public/AHKC11234.xdp
*Without a valid PracID, Dr. McGaw is not able to bill for the appointment. If you are having trouble obtaining a PracID, please contact us.
Your referral should include the Patients Personal Healthcare Number (PHCN).
Your referral should include the Patients Date of Birth (DOB).
Your referral must contain a method of contact (phone number, email address) for the patient. We recommend email as it is the best way to communicate.
Your referral details must be fully completed and specify a reason on why you would like to refer the patient to Dr. McGaw.
If you deem the Consultation Request to be Urgent, you *must* indicate that on the referral AND clearly explain the reason for urgency in the details.
Once the referral is completed, you can send it by one of the following methods:

By Faxing a copy of the completed referral to 1-855-795-4566
Once the referral is received, the referral will than be processed and sent for triage. You will be sent a Notice of Referral Received by email or fax within 10 days with any additional steps or information we may require.
The referral triage process may take a while as Dr. McGaw receives a high number of referrals.
Once the referral has been triaged, it will be assigned an appointment by the Scheduling Coordinator.
Important. Please never direct the patient to contact us regarding the status of a pending referral. We may not be able to provide this information to them.
