FORMS

Registration forms can be downloaded and filled out. Completed Registration Forms can be emailed to Margaret Jackson- margaret.jackson@onchaminahos.ca or Loran Cardinal - loranlc@sledu.ca

Registration forms must be filled completed by the Parent. If you are the Guardian of the child, you need to provided legal documentation to support this. Otherwise the parent must fill out the form. 

Please provide a copy of the Birth Certificate, Alberta Health Care Card and Status Card.  (These can be brought to the school and we will copy for you)
