Medical Forms & Policies

Annual Medical Forms (Require Doctor’s signatures)

• • Annual Medical Forms: If your child needs medication on campus (whether prescription or over-the-counter), you are required by the Diocese of Charlotte Catholic Schools Office to submit a NEW Medication and Physician Authorization Form and/or Asthma Treatment Form to the school each year. The medication must be provided to the school in its original packaging and labeled in a ziplock bag. We recommend that you have these completed and turned in at the start of the school year; we require they are turned in order for a school employee to provide medication. Sports Physical (Require Doctor’s signatures)

• • Sports Physicals: If any of your children plan to play a sport this school year, they must have a current sports physical on file. The physical must have been completed within a year from the date of the first practice. We recommend that you have these completed and turned in at the start of the school year; we require they are turned in by the date of the first practice. Overnight Field Trip Forms- MIDDLE SCHOOL ONLY (also require Doctor’s signature)

• Overnight Field Trip Forms: All middle school students will attend an overnight field trip (if permitted under COVID-19 precautions). Rising seventh graders will explore the mountains in September, rising eighth graders will venture to D.C. in April, and rising sixth graders will soak in Marine Studies at the coast in May. Overnight field trip forms required by the Diocese of Charlotte are extensive and require a Doctor’s signature. We recommend that you have these completed and turned in at the start of the school year; we require they are turned in two weeks prior to the overnight field trip.
