Medical Forms & Policies

Annual Medical Forms (Require Doctor’s signatures)

If your child needs medication on campus (whether prescription or over-the-counter), you are required by the Diocese of Charlotte Catholic Schools Office to submit a NEW Medication and Physician Authorization Form and/or Asthma Treatment Form to the school each year.  The medication must be provided to the school in its original packaging and labeled in a ziplock bag.  We recommend that you have these completed and turned in at the start of the school year; we require they are turned in order for a school employee to provide medication.  Annual medical forms are available at these links:

Sports Physical (Require Doctor’s signatures)

If any of your children plan to play a sport this school year, they must have a current sports physical on file.  The physical must have been completed within a year from the date of the first practice.  We recommend that you have these completed and turned in at the start of the school year; we require they are turned in by the date of the first practice.   The Sports physical is available at this link:  Sports Physical

Overnight Field Trip Forms - GRADE 8 ONLY

8th grade students will attend an overnight field trip. 

The following two forms must be completed for ALL students;  the Medication Authorization Form requires a Doctor’s signature.  We recommend that you have these completed and turned in at the start of the school year; we require they are turned in two weeks prior to the overnight field trip.   Overnight Field Trip Forms are available at this link:
