Student Physical Forms are required for ALL new students (PK-8th grade) as well as current students entering Kindergarten and 6th grade. The Student Physical Form must also be provided by all student-athletes annually. Physical exams must be within 12 months prior to the first day of school.
If your child needs medication on campus (whether prescription or over-the-counter), you are required by the Diocese of Charlotte Catholic Schools Office to submit a NEW Medication and Physician Authorization Form and/or Asthma Treatment Form to the school each year. The medication must be provided to the school in its original packaging and labeled in a ziplock bag. We recommend that you have these completed and turned in at the start of the school year; we require they are turned in order for a school employee to provide medication. If any of your children plan to play a sport this school year, they must have a current sports physical on file. The physical must have been completed within a year from the date of the first practice. We recommend that you have these completed and turned in at the start of the school year; we require they are turned in by the date of the first practice. 8th grade students will attend an overnight field trip.
The following two forms must be completed for ALL students; the Medication Authorization Form requires a Doctor’s signature. We recommend that you have these completed and turned in at the start of the school year; we require they are turned in two weeks prior to the overnight field trip.
