There are Changes in Premiums for the 2026 Calendar Year
These changes in benefit premiums for members, spouses, and their dependents are managed by the Employee Group Insurance Division (EGID). Please review your life insurance as it may have increased in cost or decreased in coverage amount.
Flexible Benefit Allowance
The flexible benefit allowance will remain $707.00 per month or $353.50 per pay period. (The flexible benefit allowance is a state paid statutory benefit for covering the cost of health insurance over a twelve (12) month period.)
Enrolling a Spouse or Child(ren)?
If you are enrolling a spouse or child(ren), you must have their birthdate and social security number available to enter into the system. We must have a marriage license to add a spouse and birth certificates for any children that you are adding. The deadline to submit the dependent documentation to HRBenefits@okcps.org is October 31, 2025.
Tobacco Free Attestations
If you are enrolled in the HealthChoice High or Basic plan and wish to stay enrolled in that plan, you must complete the online Tobacco-Free Attestation for plan year 2026 by Dec. 31, 2025. This does not apply to HealthChoice High Deductible Health Plan members unless you are moving from this plan to another HealthChoice plan. Please make sure to list OKCPS as the employer and not your school site or department. Your name must match your insurance card.
Health Plans
Community Care
Calendar year out-of-pocket maximum for family increasing to $10,000.
Office visits, allergy testing, and mental health or substance use disorder outpatient visits decreasing to a $25 copay/PCP.
Hospital inpatient, maternity hospital admission, mental health or substance use disorder inpatient, occupational or speech therapy inpatient, physical therapy inpatient, and inpatient bariatric surgery copays increasing to $400/day and a maximum of $2,000/admission.
Hospital outpatient copay increasing to $350/day.
Emergency room copay increasing to $300/admission.
Pharmacy copays:
Decreasing for 90-day supply of Tier 1 preferred generic, Tier 2 preferred brand, and Tier 3 non-preferred brand or generic drugs.
Increasing for 30-day supply of Tier 4 specialty drugs
HealthChoice High Deductible Health Plan
The cost for SwiftMD visits will be $0.
Dental Plans
Sun Life Dental
Preventative rewards were added.
Vision Plans
There are no significant plan changes among the vision plans.
Life Plans
Voya will allow new participants to elect up to $25,000 in supplemental life insurance without needing a life insurance application. Current participants will be able to elect an additional $25,000 up to $150,000 without needing a life application.
If you have Voya Supplemental life coverage, be sure to review your rates during Open Enrollment, as some benefits may decrease once you turn 65.
If you need to update your beneficiary information for Voya, please complete the Voya Beneficiary Form and send it to HRBenefits@okcps.org
Life Insurance Applications
If you are increasing EGID life insurance, please print and mail the Life Insurance Application to EGID, Attn: Member Accounts, P.O. Box 11137, Oklahoma City, OK 73136-9998, email it to: EGIDMail@ohca.ok.gov, or fax it to 405-717-8939.
If you are enrolling in or increasing your Voya life insurance and need to submit a life insurance application, please print and mail the Life Insurance Application to ReliaStar Life Insurance Company, P.O.
