Note: Physical copies of these forms will be distributed to your child’s teacher to bring home.
Even if your child doesn't take daily medication, please fill out the top section of this form. This gives our school nurse permission to administer over-the-counter medications (like Advil or Tylenol) if needed while at camp.
Waiver for Eagles Crest Ropes Course
Even if you or your child do not plan on participating in the ropes course, please still complete and submit this form (simply note at the top that permission is NOT granted).
Many students change their minds once they arrive at camp and see the activity! Please note: Students without a signed form on file will not be allowed to participate under any circumstances.
