Post Test - DMV Information

Post Test Information:

After successfully completing the online knowledge test, the receipt you receive is NOT a Learner Permit. You must bring the following items to ANY Department of Motor Vehicles issuing office to complete your application for a Learner Permit:

• The Online Knowledge Test Receipt

• Completed Application for Driver License (MV-44)

• Required Proof of Identity and Date of Birth

• Proof of Date of Birth

• A Social Security Card

• 4 Points of Acceptable Proof of Name

DMV staff will administer a vision test before the Learner Permit will be issued.

You CANNOT apply for a Learner Permit until you are at least 16 years of age.

All DMV issuing offices locations are listed in the Driver's Manual. You will need to complete the following forms before going to the DMV office. Click on form below. If you do not have access to a computer, ask the Test Proctor for a printed packet of forms.

• MV-44 Application for Driver License or Non-Driver ID card (and Permit)

• ID-44 Lists of Acceptable Proofs of Identity

• MV-45 Statement of Identity by Parent/Guardian
