These forms are for Media Lab rental equipment only. DES students wishing to rent a laptop or iPad will need to get that form from their instructor.
The loan agreement is to be filled out once a semester. The request form is to be filled out for each piece of equipment being requested.
An Oakland email is required for completing Forms. Requests for outside organizations will not be granted.
