What happens if I lose my four digit code or don't write it down for the next person in the workflow? 

• If this happens, please email sh8527@nyu.edu. We will help you locate the proper code to pass along to the next individual in the workflow. 

What happens if I have started filling out the module, but I receive new information and want to adjust my answers but retain others? 

• If this happens, please email sh8527@nyu.edu. We will retrieve the information you need and help you adjust the module. 

I am confused by the need for a four-digit code. Can you explain this? 

• Typical Workflow

  • Sexual Harassment

    • IO/JA complete facts, findings, and recommendations module. Information is retrievable via code number. 

    • Code number provided to commander to complete module (with JAG consultation)

  • Sexual Assault

    • JA completes module with CID Investigation Information. Module with information is retrievable via code number. 

    • Code number provided to commander to complete module (with JA consultation)

• Alternate Workflow

  • Commander selects “No CID Information Entered” option and completes full set of questions themselves (with JA consultation)
