1. Print out DOR 203, DOR 260 and NVUSD Release forms.
2.Please sign and complete DR 203 and DR 260, and the NVUSD Release of Information forms.
3. Once signed and complete, please return form to Specialized Vocational Services (SVS) for processing.
4. Once processed, students will meet with DOR Counselor for intake meeting to determine appropriate vocational services.
