Initial Parent Letter, Student Information Sheet, CCSD Permission Slip, Medical Form, Insurance Waiver

Each of the following forms MUST be completed and turned in to reserve a spot on the Pali Field Trip. 

*These forms were all in the packet with the yellow student information form on top that went home at the initial parent meeting. If you have already completed and returned the packet, we don't need anything else from you at this time*

 The McCaw school nurse will be in contact with all parents of students who are bringing medication (both prescription and over the counter) as indicated on the Medical Permission Form to go over protocol and requirements for the medications. This will happen in the Spring when the trip gets much closer.
