Five (5) DTaP/DPT (Minimum age: 6 weeks)
- 5th dose not needed if 4th dose given on or after the 4th birthday
- Final dose at least 6 month after the previous dose
Four (4) Polio/IPV/OPV (Minimum age: 6 weeks)
- 4th dose not needed if 3rd dose given on or after the 4th birthday
- If both OPV & IPV were administered as part of a series, a total of 4 doses are needed regardless of child's age
- Final dose at least 6 months after previous dose
Two (2) MMR (Minimum age: 12 months)
- 1st and 2nd dose must be separated by at least 4 weeks
- 2nd nor required until on or after the 4th birthday
Two (2) Hepatitis A (Minimum age: 12 months)
- 2nd dose must be given at least 6 months after the 1st dose
- May be in progress
Three (3) Hepatitis B (Minimum age: birth)
- 1st and 2nd dose must be separated by 4 weeks
- 2nd and 3rd dose must be separated by at least 8 weeks
- 3rd dose at least 16 weeks after the first dose. (Minimum age for 3rd dose: 24 weeks)
Two (2) Varicella/Chicken Pox (Minimum age: 12 months)
- 1st an 2nd dose must be separated by 4 weeks
- If 2 doses of Varicella are administered at the correct interval, a 3rd dose is not required
- History of Chicken Pox disease will ONLY be accepted by a physician's note.
Incoming Kindergarten - DO NOT WAIT to receive your immunizations. Registrations without up-to-date immunizations will not be accepted.
NEVADA STATE LAW REQUIRES THAT ALL STUDENTS BE IN COMPLIANCE BEFORE ENROLLING IN SCHOOL. YOUR CHILD WILL NOT BE ALLOWED TO ATTEND SCHOOL IF THEY ARE NOT IN COMPLIANCE.
