1. RMA / Aktivate Athletic Account (Completed)
   Please review the instrctuions before completing. 

2. Student Physical Examination Form
   Must be completed and signed by a licensed physician and uploaded to RMA or submitted to the Athletic Office. Only upload the completed LAST PAGE (Medical Eligibility Form).

3. Authorization and Release for Student Information (Athletic Packet)
   Parent/guardian and student signatures required; submit to the Athletic Office.

4. Requirements to Participate in Sports (Reviewed)
   Student must meet academic eligibility, residency, and zoning requirements.

5. Students Must Maintain Above 2.0 GPA
   Minimum 2.0 GPA from the previous semester (for 10th–12th grade) 

Transfer Students might be required to provide the following information

• Two Proofs of Address

• Must be a WATER, POWER, or GAS bill

• Must be submitted in PDF format

• Address must match Infinite Campus (IC) and RMA exactly

• Official Transcript (Transfers Only)

• Proof of Abandonment of Previous Residence (If Applicable)

• Custody / Guardianship Documentation (If Applicable)

• Zone Variance Approval (If Applicable)

To be eligible, students must:

• Be officially enrolled at Arbor View High School

  • IF you attend a Charter/homeschool/online you must complete the Online Registration for partial enrollment for sports through Infinite Campus

    • Pay the off campus fee in the fall 

• Meet CCSD and NIAA academic eligibility requirements

• Meet residency and zoning requirements

Failure to meet these requirements may result in delayed or denied eligibility.

Once eligibility is approved, students are expected to follow the practice and participation expectations below. 

• Student must maintain above a minimum 2.0 GPA.;3

• Practices take place after school

• Players may not participate until eligibility is fully approved

• Attendance is expected once participation begins

• Equipment and team expectations will be communicated by the coaching staff

Schedules and deadlines must be followed in order for student-athletes to remain eligible for participation.
