RMA / Aktivate Athletic Account (Completed)
Student and parent/guardian must create and complete the RMA / Aktivate account with all required electronic signatures. Please read the instructions before completing.Current Student Physical Examination Form
Must be completed and signed by a licensed physician and uploaded to RMA or submitted to the Athletic Office. Only upload the completed LAST PAGE (Medical Eligibility Form).

Team Fee Paid in Full by July 1st - $500 (Non-Refundable)
Fundraising CAN cover all of Team Fee
*Only money raised from the Redline Fundraiser can be used towards the team fee*
Raise $300 - get 50% off Team Fee
Raise $600 - get 100% off Team Fee

Payment of the team fee does NOT guarantee participation in games.
Participation may be limited due to academic ineligibility, injury, or disciplinary action.
Team Fee Explanation :
The team fee covers operational costs throughout the season including services, equipment, transportation, and other program expenses.
The team fee is non-refundable.

IMPORTANT - All eligibility requirements must be completed in RMA (Aktivate) before participation is allowed.
Create or log into an Aktivate account at aktivate.co,
Start a new registration and select Arbor View High School (NV)
Create or select the student-athlete profile
Use the student’s full legal name (no nicknames)
Address must match Infinite Campus
Select the 2024–2025 school year and all sports the student may want to try
Enter guardian information
Guardian 1 must match the primary guardian in Infinite Campus
Complete health insurance and medical questionnaire
Complete the NIAA Tryout Checklist & High School Questionnaire
Parent and student must e-sign
Some answers may trigger a transfer review (this is normal)
Freshmen or new students: complete guardianship and residency verification
May need proof of address and custody documents
Review and e-sign all electronic documents
Download, complete, and submit the NIAA physical form
Physical must be within the last 12 months
Upload or turn in the Medical Eligibility Form
Pay the $40 Aktivate fee (once per school year)

To be eligible for participation in school athletics, students must:
Complete and submit all required information through RMA / Aktivate
Meet academic eligibility requirements (GPA and credit requirements)
Meet residency and zoning requirements
Provide additional documentation if applicable (transfer, hardship, or zone variance)
Ensure all required documents are completed and approved before participation
