The Prenatal Care Coordinator will work directly with Medicaid-eligible pregnant individuals to ensure they access the necessary prenatal care and services as part of the PNCC program. This role involves conducting initial assessments, connecting members with the QP to develop initial care plans and receive health education/nutrition services, updating care plans, coordinating services, and maintaining accurate records. The ideal candidate will have a passion for maternal and child health, a commitment to supporting diverse families, and the ability to work collaboratively with both families and community partners.
Outreach: Identify and engage eligible, low-income pregnant individuals in need of prenatal care services.
Initial Assessments: Complete comprehensive assessments to evaluate physical, social, and emotional needs, and determine eligibility for PNCC services.
Ongoing Care Coordination: Regularly follow up with members, monitor progress, update care plans, and provide referrals to community resources.
Health Education: Assist the member in enrolling in classes or setting up appointments with the QP to receive health education and nutrition counseling on topics such as smoking cessation, alcohol use, prenatal nutrition, breastfeeding etc.
Documentation: Maintain detailed and accurate records of all contacts, assessments, care plans, and services provided.
Collaboration: Work closely with medical professionals, community organizations, staff and collateral contacts as part of the member’s care plan to ensure the member’s needs are met.
Compliance: Ensure adherence to ForwardHealth and Medicaid guidelines, including service documentation and timely completion of assessments and care plans.
Education: High School Diploma required; Associate’s or Bachelor’s degree in health, social work, or related field preferred.
Experience: Previous experience in prenatal care coordination, maternal health, case management, or related fields is highly desirable.
Skills:
Strong organizational, communication, and computer skills.
Ability to engage and build trust with individuals from diverse socio-economic, racial, and ethnic backgrounds.
Knowledge of child development, maternal health, and community resources.
Proficiency in Google Docs and Google Sheets.
Bilingual (Spanish or Hmong) is preferred but not required.
Other Requirements:
Must have a valid Wisconsin Driver's License, reliable transportation, and adequate auto insurance.
Must pass background check as per program requirements.
Ability to work independently and manage a caseload of 10-15 clients.
The qualified professional will work directly with Medicaid-eligible pregnant individuals to ensure they have access to the necessary prenatal care and services through the PNCC program. This role involves the development and implementation of personalized care plans, supervision of Care Coordinators, and the creation and delivery of educational curriculums on essential topics such as health and nutrition, safe sleep, child development, breastfeeding, and both physical and mental health, etc. The qualified professional may also be required to manage high-risk cases as needed, providing additional support to ensure the well-being of both the mother and the baby. As part of a collaborative team, you will have the opportunity to develop your skills and grow within the social services field.
Key Responsibilities

Education: Candidate must have One of the following
A dietitian certified or eligible for registration by the Commission on Dietetic Registration of the American Dietetic Association with at least 2 years of community health experience
A registered nurse with at least 2 years of experience in maternity nursing or community health services or a combination of maternity nursing and community health services
A bachelor's degree and 2 years of experience in a health care or family services program
A health educator with a master's degree in health education and at least 2 years of experience in community health services
Experience:
Two years of experience coordinating services for at-risk or low-income women and pregnant people.
Expertise in health education and nutrition counseling for high-risk pregnant women, either through formal education or at least one year of practical experience.
Skills:

We are seeking a reliable and proactive Administrative Assistant to support various operations within our organization. This entry-level position offers a flexible schedule and is ideal for individuals who are looking to gain experience in administration, marketing, and community outreach. The role includes assisting with administrative tasks such as managing QuickBooks, payroll, filing paperwork, creating marketing materials, and supporting marketing efforts. You will also be involved in managing inventory, processing documents, and helping the team with various other tasks. As the role grows, there will be opportunities to take on additional responsibilities and hours.
Key Responsibilities:
Administrative Support: File paperwork, organize office records, and manage inventory.
Marketing & Outreach: Assist in creating flyers, promoting community events, and getting the organization involved in local events and partnerships.
QuickBooks & Payroll: Support in managing QuickBooks for basic bookkeeping tasks and assist with payroll processing.
Team Coordination: Help set up team meetings, send reminders, and ensure smooth communication across the team.
Cleaning & Organization: Perform general cleaning duties and ensure the office is organized as needed.
Miscellaneous Duties: Assist with processing various paperwork and other administrative tasks as needed to support the team.
Position Details:
Schedule: 10 hours per week, split across 2 days of your choice (one shift from 8:00 AM – 2:00 PM, and one from 2:00 PM – 7:00 PM).
Future Growth: This position may require additional hours as the organization grows.
Compensation: Starting pay of $15-$18 per hour, depending on experience.
