If your child needs transportation to or from an address other than your home address, please complete the UPK Childcare Transportation Request Form and return it to the Transportation Department.

If this address changes after you receive your UPK acceptance letter, please call the UPK office at 315-218-2056 to confirm that the new address is still within your child’s UPK site attendance zone.
