MEDICATION ORDERS

Any medication to be taken in school(prescription or over-the-counter) must be brought to the Health Office by the parent/guardian in the original container. A written statement to dispense medication must be on file from a physician/dentist that includes the name of the medication, the dosage and frequency of administration. The parent/guardian must sign a written request for the school nurse to dispense the medication. Any medication brought to school must be picked up by the parent/guardian prior to the end of the school year. Medication not picked up will be discarded in the appropriate manner.  Please see below for the North Syracuse Junior High medication forms. Asthmatic/ Allergic students who need to self-carry/self-medicate for acute symptoms must have written authorization from both the students physician and parent.
