Students with Asthma need to have medication orders from the physician and parental permission for each school year. If a student is involved in athletic/co-curricular activities the order must state that the student can carry and self administer the inhaler. If a nebulizer treatment is ordered parents must supply the medication and tubing needed to deliver the treatment to be kept in health office. The form for medication below can be printed to have the physician complete the orders.
