REGISTRATION FOR ATLETICS AND EXTRACURRICULAR ACTIVITIES IS ONLINE

Students must register under the school they attend.
Registration starts 30 days prior to the sport or extracurricular start date
A copy of a current physical must received in the health office or uploaded to Final Forms prior to a student being approved to tryout or participate.
Physicals are valid for one year from the date of the exam. Students who require medication for asthma, or food/insect allergies must have a current physician order or physician signed Allergy Action Plan and parental permission on file in the health office.
Medication orders or Allergy Action Plans need to be renewed each school year.
The physician order must state the child may carry and self- administer the medication.
Athletic Placement Process for 7th graders playing a JV/V level sport
STEPS FOR ATHLETES TO PARTICIPATE IN UPPER LEVEL SPORTS
Step 1- Coaches will contact parent/student athlete and give paperwork
Step 2- Parent review APP packet

Attach copy of a current physical (MUST BE WITHIN 12 MONTHS) that also includes students height/weight and tanner score (measurement for physical maturity).
Step 3- Return completed packet to Athletic Office no later than 3/3/2026.
Step 4- Student athletes will be evaluated by district Medical Director and NSJH Nurse on 3/7/2026 at 0730 am at NSJHS.
If student is approved they will move onto steps 5 and 6.
Step 5- Student athlete will complete the physical fitness testing with the Physical Education teacher ON 3/10/2026 at 2:10pm at NSJHS.
Step 6- Student will be approved in Final Forms and will begin tryouts
