It is required by New York State that students in 11th grades receive a Health Examination by a NYS licensed provider. This examination must be provided to the Health Office within 30 days of entry into school.
Students new to the NSCSD district require a Health Examination with-in 30 days of starting
Annual Health Examination is required for sports and working papers.
12th Grade Immunization:
Meningococcal Vaccine is required for all students entering 12th grade. Proof of immunization must be received no later than 14 days after the start of school.
To avoid interruption in your child's attendance at school, please consult with your child's health care provider to ensure your child is up to date on all immunizations.
Dental Certificate is requested, not required, at any time a health examination is required.
The district’s School Health Services program supports your student’s academic success by promoting health in the school setting. One way that we provide care for your student is by performing the health screenings as mandated by the State of New York. 11th graders and new students are required to have a hearing and vision screening completed.
A letter will be sent home if your student needs follow-up with your health care provider. Please call the school’s Health Office if you have any questions or concerns.
