School Physicals- Wednesday November 13 

Required for all 11th graders and new students. Annual physicals are required for sports and working papers.

School physicals are being offered during school hours. .

Students will need to have Health Record and History Form completed and signed by the parent.  These can be printed from the C-NS website or picked up in the health office. 

The student then brings the completed/signed form to the health office and we will schedule them a time for their physical.
