Maintaining the security and confidentiality of assessments is critical for ensuring fair and equal testing opportunities for all students.
Testing irregularities can occur before, during, and after testing. Conduct that violates the security and confidentiality of a test is considered a serious testing violation. Procedural testing irregularities are less severe, more common, and typically the result of minor deviations in testing procedures. A routine test administration training is the best way to avoid testing irregularities, especially after updating policies and procedures.
All testing irregularities must be reported to the DTC in the Office of Policy, Planning Evaluation and Testing. Each person participating in the Assessment is responsible for reporting any violation or suspected violation of test security or confidentiality. Staff should notify their STC if they witness an irregularity or suspect one has occurred. STC must ensure that all testing personnel know their obligation to report testing irregularities. To report testing irregularities, complete the irregularity form.
This form is used to submit all assessment irregularities. Please don't use this to submit other documents.

Missing Students: If a student still needs to be uploaded to Kite, submit their name using the form.
Teacher identification: The for will be used to add any DLM teacher that was not captured in the upload
DLM Certifications: After completing the DLM required training, submit your certification via the form.

Performance Matters Account Request: Use this form ONLY for administrations that do not have PowerSchool Access. Teachers automatically have access to the test site.
Performance Matters Irregularity Form: Complete the form if you encounter irregularities that occur during the testing window.
