ICC Sign-Up Paperwork

Parents/Guardians should complete the attached forms to register students for the ICC after school program at Hawkins Street School. Once students are enrolled in ICC they can join the after school band program and receive their rental instrument to be used for the rest of the school year.

Instrument Selection Form

Complete this form to let Mr. Kertesz know what instrument you would like to play in the ICC after school Band. You must sign-up for ICC before signing up on this form. If you need the sign-up paperwork please see Mr. Kertesz or Ms. Ware in the main office.

Instrument Rental Agreement

After receiving confirmation of your instrument for ICC Band, please complete this form for Mr. Kertesz.

You will not be able to receive your instrument until a signed copy is returned to Mr. Kertesz.
