Incident Report Protocols

Incident forms can be found through the YNS HUB "Home" page (see screen shots below)

Select the Plaistow location when submitting an incident report to obtain the correct google form to complete.  

The following scenarios call for an incident report to be documented;

• Property Damage 

• Theft 

• Member Relations (fighting, conduct issues, safety concerns, suspicious behavior)

• Staff Medical (medical situations, as well as situations where we give out an ice pack/band aid, etc.)

• Member Medical (medical situations, as well as situations where we give out ice pack/band aid, etc.)

• Staff Concerns

Be sure to obtain the following key information regarding the incident and submit the incident form as close to the time of the event as possible when the situation has been handled.

• Date of Incident

• Approximate Time of Incident

• Location of Incident

• First & Last Names of Individuals Involved/Reporting Incident

• As many details regarding incident as possible

• If individual is under 18, phone number to notify parent/guardian

In the case that law enforcement is called/comes to the Y, or if the individual is taken from Y in an ambulance, you MUST immediately notify (call) your supervisor, Laura Nicholson, when you are not directly handling the incident.

If the incident is NOT an emergency that requires the police, fire department, or ambulance to be called to the Y or come to the Y, you can text your supervisor or send an email to inform of the incident.  Your supervisor will ensure the correct individuals are notified in the case of an emergency by following the chain of command.
